Hello, I'm Diane Schade. I'm a highly qualified board certified Geriatric and Palliative Nurse Practitioner with a decade of community healthcare experience, serving palliative and geriatric patients in their homes. 

My career began when, as a nursing student, I took an elective course on palliative and end-of-life care. I knew immediately that's what I wanted to specialize in. Being a nurse is more than a career for me. It's a calling.

Getting to know my patients and their families in their most comfortable surroundings is an absolute privilege. I feel honored to be invited into a patient's home when they are not feeling well and at their most vulnerable. That trust is not taken lightly and it fuels my desire to be an educated and passionate advocate for the people I work with.

I understand that different therapies and wellness treatments are sometimes desired and often required to promote emotional and physical comfort. My goal is to provide my patients and their families with resources for any and all services they might want including:
